Reportable incidents

A reportable incident is an act or event that has happened, or alleged to have happened, in connection with delivering NDIS supports or services.  

Registered NDIS providers must notify the NDIS Commission of all reportable incidents.

This includes incidents you have recorded and responded to within your own incident management system.

Timeframes to notify us

Reportable incident

You MUST notify us within:

(Timeframes are calculated from when a registered NDIS provider became aware of the incident) 

Death of a person with disability. 24 hours
Serious injury of a person with disability.24 hours
Abuse or neglect of a person with disability. 24 hours
Unlawful sexual or physical contact with, or assault of, a person with disability.24 hours
Sexual misconduct against, or in the presence of, a person with disability, including grooming of the person for sexual activity.24 hours
Use of a restrictive practice that is unauthorised by your state or territory, or does not follow a behaviour support plan for the person with disability. 

5 business days

If the incident has resulted in harm to a person with disability, it must be reported within 24 hours. 

If you do not report an incident within the timeframes, this might result in an infringement notice or other compliance actions.

How to notify us

  • Log in to the NDIS Commission Portal
  • Go to ‘My Reportable Incidents’
  • Complete and submit the following forms.

Immediate Notification Form

You need to submit this form within the specified timeframe. In most cases, this is within 24 hours of becoming aware of a reportable incident.

5 Day Form

You need to provide additional information and actions you’ve taken. Submit this form within five business days of becoming aware of a reportable incident.

This is the only form needed for incidents involving unauthorised use of a restrictive practice that has not caused immediate harm to a person with disability.
